Continuing Education Certificate Replacement Request
If you have misplaced your Continuing Education Certificate and need to reorder one, please submit your request electronically.  Members are eligible for a free certificate replacement.  Members must log in to get the $0 rate.  Non-members must pay a fee of $10 before the certificate will be emailed to them.  If you have any questions, please email Taylor Sambrano, at tsambrano.naswtx@socialworkers.org or call 512-474-1454.

This information is required to obtain a copy of your certificate. Members will receive a free replacement; however, non-members must pay a fee of $10 to obtain a copy.

If you cannot complete this request electronically, please email your request to Taylor Brewer at tbrewer.naswtx@socialworkers.org and she will take your payment information over the phone. This system will generate an automatic receipt if completed online.
